How to Refer a Patient

To refer a patient please call our clinic at: (919) 668-5580. To assist us with scheduling a patient please fill out the adult referral sheet (PDF) or congenital referral sheet (PDF) and fax it to us or have the following information ready when you call:

1. The name and phone number of the patient.
   
2. The indication(s) for the exam.
   
3. The name and phone number of the referring physician.
